Immunizations:

All students must be properly immunized to attend on the child's first day of school. 

Requirements vary by grade level, so to see if your child is up to date, see the state requirements below. State law and health regulations require students to be properly immunized and provide verification to attend school, unless they are exempt.

If your child needs to get his/her immunizations they are available through your private doctor, local resources, and the Warren County Health Department. Call 636-456-7474 for more information. They are located at the following address.

A parent or legal guardian must accompany the child to any clinic sponsored by the Warren County Health Department to sign the permission form for the immunizations. In the event a parent or legal guardian cannot be present, please call the Warren County Health Department at 636-456-7474. You must also take your child’s updated immunization records with you to the clinic. Employment and services are provided on a non-discriminatory basis.

Medications:
The parent/guardian must provide the district with written permission to administer the medication before the district will administer the prescription medication to the student. The prescription label will be considered the equivalent of a prescriber's written direction, and a separate document is not needed.

Health Screenings:

The Warrenton School District provides vision and hearing screenings for students.  If you would like for your child to be screened, please contact your school nurse.

Sports:
A completed MSHSAA Physical form must be completed and on file with your child's school annually before students may participate in middle or high school sports.  Your school office can provide you a form, or download a copy from the Health Services forms section.
